Referrals can be made by calling the ABVI office directly at 843.723.6915 or emailing info@abvisc.org. One of our Client Services Specialists will reach out to get basic information, a copy of an eye report, and complete the client application process. Doctors can also submit an eye report directly to ABVI via fax at 843.577.4312.

Self-Referrals

Doctor Referrals

Providers, please complete a report of eye examination for the potential client. In addition to the vision information, please include the patient’s name, date of birth and best contact phone number. Once finished, scan to info@abvisc.org or fax to 843-577-4312. A team member in our Client Services Department will reach out to the client to complete the remaining part of the application within three business days.

VisionRefer

Doctors also have the option of using VisionRefer, a secure, HIPPA compliant patient referral system. This system is designed to facilitate the patient referral process to accredited vision rehabilitation specialists such as ABVI. A doctor’s office enters a patient’s basic contact information, including name, address, phone number, email and birthday, as well as any other pertinent documentation into the web-based referral form. Once the form is received, an ABVI staff member will contact the patient within three business days to discuss their individual needs and which services may help maintain or increase their independence.
